CCD benefit-eligible faculty and staff may use this form to take advantage of our tuition reimbursement program. Reference SP 3-60e for additional information regarding this benefit.

Please forward the completed application that includes the employee's and supervisor's signatures to HREP@ccd.edu. The completed application must be returned to CCD HR by the registrar’s census date of the term of attendance at the College providing the class.
