Change of Information Request

The Change of Information Request is submitted by a student who needs one of the following updated/corrected on their CCD account: name, preferred first name, social security number, address, phone number, email address, date of birth, gender, ethnicity, race, citizenship. Many change requests, such as a name or social security changes, require supporting documentation before they can be processed by the Office of Registration & Records.

Change of Information Requests are processed within three to five (3-5) days after a complete request form is received.
