FERPA Release & Student Reference Request

The FERPA Release for Faculty and Staff Third Party Communication is a release form used for students to authorize faculty and staff outside of the Registration & Records and Financial Aid offices to serve as a reference for the student or to release their student information to a third party individual or agency.
