Intake Form

The Intake Form is used by the staff at the front desk in the Academic Advising Center to schedule and monitor students who wish to see their advisor in the Center. The form is used to document the advising session between the advisor and the student. The student will receive a copy and documentation is entered into the CCD Banner system.
