Photo & Video Release Form

The Photo & Video Release Form should be completed by individual students or employees in order to give permission to use a photo on/in any college marketing. Students must be above 18 years of age. The group form may be used when photographing in a classroom setting, or similar.

If there is an event where photography is taking place, the Photo Release Sign should be visible.
