Release of Financial Aid Information

This form is most commonly used to allow you to qualify for public benefits like SNAP (food stamps) or subsidized housing and to apply for financial aid at another school. 

Please be advised that our institution can no longer send release of information requests to anyone other than the student.  The Department of Education's (ED) Privacy and Technical Assistance Center (PTAC) has confirmed that FAFSA data cannot be released by the institution to a third party even with the student's written authorization.
