Veteran Notice of Student Responsibility

This form is to be submitted by VA students who are using Veterans benefits for tuition at CCD. CCD Veteran Support Services will need the VA Enrollment Form along with the Veteran’s Notice of Student Responsibility Form below in order for our office to submit your enrollment status to the VA Office for processing.
