Waiver of Liability for Domestic International Travel Student Form

This form is used when a student is traveling as a representative of the college releasing the College of liability for their participation in the activity. This must be signed by the student and, if the student is under the age of 18, a witness and parent or guardian must also sign. The form needs to be maintained by the department sponsoring the travel.
